Nurse Navigators
Dedicated nurses working as advocates for patients and families to coordinate all aspects of care. They provide support and serve as a consistent point of contact to help navigate the many medical care decisions and appointments.
Survivorship
Cancer survivorship starts at diagnosis and continues throughout the patient’s life. The Meridian Cancer Care survivorship plan ensures coordinated patient-centered care between primary care providers and specialists.
Genetic Counseling
Family history or genetic factors can increase an individual’s risk for cancer. Meridian Cancer Care’s genetic counseling program provides risk assessment and possible testing so you can take preventive steps and protect your health.
Support Groups
Meridian Cancer Care offers many types of support groups that serve cancer patients and their significant others. The group members find solace in sharing their feelings, experiences, and concerns with those who can best understand them — others who have gone through similar experiences.
Palliative Care
Palliative care can include anything from relief from symptoms, pain and stress, to assistance with insurance, support groups and advance care planning. Palliative care aims to improve the quality of life for people facing a serious illness.
Home Health and Hospice
Meridian At Home provides comprehensive cancer care provided in your home including personal assistance, visiting nurses and medication management programs. In addition, Meridian At Home offers support services to patients and their families for end-of-life comfort and care.
Rehabilitation
A network of facilities throughout the region comprised of a team of health and fitness professionals who offer comprehensive services to help cancer patients regain a sense of hope and control over their health including issues related to fatigue, weakness and lymphedema.
Social Work
Case management and licensed clinical social workers counsel and support patients and their families as they transition from inpatient to outpatient services to ensure it moves as smooth as possible.
Integrative Medicine
Cancer Options and Possibilities for Everyone (C.O.P.E.) consists of a comprehensive selection of wellness services and options that take care of the whole person – Mind, Body, and Spirit. The COPE program offered through Meridian Fitness & Wellness includes Therapeutic Yoga Classes, Life Fitness Fights Fatigue – Group Exercise Class, Massage and Reflexology, Nutritional Counseling, and Medical Fitness Membership.
