Meridian At Home Remote Monitoring Pilot Program Receives Quality Improvement Award

Congestive heart failure patients monitored from the comfort of their own homes  

Neptune, NJ - The New Jersey Hospital Association recently presented Meridian At Home with the first-ever Excellence in Quality Improvements Award for their Congestive Heart Failure (CHF) Pilot Program which took place in partnership with Ocean Medical Center in Brick. The award was presented in the post-acute care category at their 92nd Annual Meeting.  

The program monitored a pilot group of patients who suffered from CHF and were subsequently treated at Ocean Medical Center. The program provided a remote monitoring scale for patients to weigh themselves on each day after returning home. Their weight was then electronically transferred to a nurse at Ocean Medical Center for review and tracking. Monitoring body weight in CHF patients can prevent readmission to the hospital in the long-run. If anything alarming was sent back, action could be taken immediately.   

"Rapid weight gain in CHF patients can signify water retention, which is a tell-tale sign that something is wrong with operation of the heart," says Sandra Elliott, director of Consumer Technology and Service Development at Meridian Health. "This program offers great benefits to the patient. They can remain under the watchful eye of the hospital from the comfort of their own home, and the patient's health can be monitored and tracked in an extremely accurate and safe manner."  

In addition to at home remote monitoring, patients also received education, follow-up, and support from medical professionals. Through daily monitoring of these patients re-admission rates dropped, with the percentage falling from 14.93 percent to 4.84 percent in less than eight months.  

The New Jersey Hospital Association is the statewide advocate for its 110 member hospitals, its 300-plus post-acute members, and the patients they serve.
